Transition care is short-term support to help you recover and regain independence for after a hospital stay, helping to reduce the need for longer term support. Your care is tailored to your needs and goals and can include allied health services, nursing, and personal care.
This service includes:
- care and medication management
- clinical and therapy services
- assistance to access medical practitioners
- emergency or after-hours assistance, when needed
- help with personal affairs and social support
- continence management
- waste disposal.
This service does not include:
- ongoing care management beyond the period of transition care.

How do I get assessed for services?
An aged care assessment determines whether you are approved to access services. Using our online tool, you can check if you meet the requirements for an assessment before you apply for one.
