When to get a support plan review or reassessment

If you’ve had an assessment in the past, there may come a time when you need a Support Plan Review or a reassessment.

The assessment you had before was based on your needs at that time. But circumstances can change.

You may find that since your last assessment, your needs are different. Perhaps some daily tasks have become more difficult for you. You might be considering moving to an aged care home if you can no longer live independently at home. Or you may need more help than your current carer, family or friends can provide.

If you had an assessment in the past and weren’t eligible for government-subsidised aged care, you might be now if your needs have changed.

Depending on your situation, there are steps you can take to have your support needs reviewed.

If you already receive aged care services

If you are receiving care but find that your needs have changed, you can request a support plan review at any time through My Aged Care or from a Services Australia Aged Care Specialist Officer.  You can find your current support plan by logging into your My Aged Care Online Account.

Your service provider will then contact your assessor. Your assessor will review your support plan to ensure it meets your changed needs. If your care needs haven’t changed much your assessor might recommend some changes to your support plan. If you your care needs have changed significantly, your assessor might recommend a reassessment.

If you are Aboriginal and/or Torres Strait Islander, you can request that your reassessment is done by an Aboriginal and Torres Strait Islander assessment organisation, if one is available in your area.

If you had an assessment in the past and weren’t eligible 

Your needs may have changed since your last assessment, so you may now be eligible for aged care support. You can ask for a reassessment at any time.
